It does not cover information collected when you become a client of the Practice — that is governed separately by our HIPAA Notice of Privacy Practices and the documents you sign at intake.

  • Insurance carrier, member ID, date of birth, state (Insurance verification form only)
  • Any notes you provide

Text messaging (SMS): If you provide your phone number, we may communicate with you by text message. Messages may include appointment scheduling, session reminders, and responses to your inquiries. Message frequency may vary. Message and data rates may apply. Reply STOP at any time to opt out, or reply HELP for assistance, or contact us at (646) 374-2827. Mobile phone information will not be shared with third parties or affiliates for marketing or promotional purposes.

Email correspondence: If you choose to correspond with us via email, we may retain your email address and the contents of your email messages along with our responses.

Information collected automatically:

  • Standard web logs: IP address, the domain name of your internet service provider, operating system, web browser, referral URL, pages viewed, duration of visit, and timestamps
  • Cookies for analytics (see Section 5)

We do not intentionally collect Protected Health Information (PHI) through public website forms. Forms include a disclaimer asking visitors not to include sensitive health details.
